What should you do to maintain your health insurance?

You will need the Retiree Contact Form to update any contact and banking information. If you choose to pay by check, please select this option on the Retiree Contact Form. This form is used to keep your contact information up to date. If you have made any changes to phone numbers, addresses, banking information.

Please complete this form and return it by the following methods.

  • Mail checks to: P.O. Box 24130 New York, NY 10087-4130
  • Submit the ACH Election form to the Business Office for the ACH withdrawal option. You can mail this form to: Treasurer’s Office 507 Deer Park Rd Huntington Station, NY 11746.
  • Forms can also be emailed to: Business@wsboces.org

When are the retirement health insurance premiums due?

Yearly, a new schedule of billing, ACH withdrawal payments and due dates will be issued. The most recent schedule is outlined on the Benefits Calendar.

How do I receive Medicare Part B Reimbursement?

Pleae complete and submit the Medicare Part B Reimbursement Form. If you have any questions on completing the form please contact Benefits at 631-549-4900 X298.

Can I continue my other benefits into retirement?

Yes, you have the ability to continue your life insurance and excess major medical benefits into retirement. Your dental insurance is eligible to be continued under COBRA for up to 18 months.
